Uses of Firendia 10

• Management of chronic kidney disease (stages 3 and 4 with abnormal presence of albumin in the urine) associated with type 2 diabetes in adults

How does Firendia 10 work?

Finerenone works by blocking the action of certain hormones (mineralocorticoids) that can damage your kidneys and heart.

Side Effects of Firendia 10

• High potassium level (hyperkalemia)
• Weakness or tiredness
• Feeling sick (nausea)
• Numbness in the hands and lips
• Muscle cramps
• Decreased pulse rate
• Low sodium level (hyponatremia)
• Tiredness
• Headache
• Confusion
• Muscle weakness, spasms or cramps
• Decrease in how well the kidneys filter blood (glomerular filtration rate decreased)
• High uric acid level (hyperuricaemia)

Before Taking Finerenone

• Inform your doctor about any history of heart, liver, or kidney issues.
• Mention all medications you are taking, especially diuretics, antibacterial agents, and potassium supplements.
• Do not take Finerenone if you are allergic to any of its components; consult your healthcare provider if unsure.
• Avoid strong CYP3A4 inhibitors and grapefruit or grapefruit juice while on Finerenone.
• Use Finerenone during pregnancy only if necessary and advised by your doctor.
• Do not breastfeed while taking Finerenone as it may harm your child.
• Finerenone is not recommended for children or teenagers under 18 due to unknown safety and efficacy in this age group.

Indication of Firendia 10

Firendia 10 is used to treat patients with chronic renal damage caused by type 2 diabetes. Firendia 10 also lowered the occurrence of new-onset heart failure considerably. Chronic renal disease is defined by a gradual kidney function decline over time. Type 2 diabetes occurs when your body cannot maintain normal blood sugar levels.

Pharmacology

Finerenone is a nonsteroidal, selective mineralocorticoid receptor (MR) antagonist. It blocks MR activation by aldosterone and cortisol, reducing sodium reabsorption and preventing MR overactivation in tissues like the kidneys, heart, and blood vessels. This helps mitigate fibrosis and inflammation.

Dosage & Administration of Firendia 10

Starting Dosage:

• eGFR ≥60 mL/min/1.73 m²: 20 mg once daily
• eGFR ≥25 to <60 mL/min/1.73 m²: 10 mg once daily
• eGFR <25 mL/min/1.73 m²: Not recommended

Dosage Adjustment:

• Increase to 20 mg once daily after 4 weeks, based on eGFR and serum potassium levels.
• For those who cannot swallow tablets, they can be crushed and mixed with water or soft foods.

Monitoring and Dose Adjustment:

• Measure serum potassium 4 weeks after starting treatment. If levels are >4.8 to 5.0 mEq/L, consider treatment initiation with additional monitoring.
• Monitor serum potassium 4 weeks after any dose adjustments and throughout treatment, adjusting the dose as necessary.

Missed Dose:

• Take the missed dose as soon as possible on the same day. If it’s too late, skip the missed dose and resume the regular dosing schedule.

Drug-Drug Interactions for Firendia 10

Inform your doctor if you are taking any of the following:

• Antifungal Agents: Itraconazole, ketoconazole
• Anti-HIV Agents: Ritonavir, nelfinavir, cobicistat
• Antibacterial Agents: Clarithromycin, telithromycin, erythromycin
• Diuretics: Amiloride, triamterene
• Other MR Antagonists: Eplerenone, esaxerenone, spironolactone, canrenone
• Trimethoprim or a combination of trimethoprim + sulfamethoxazole
• Potassium Supplements
• Cardiac Drugs: Verapamil
• Rifampicin (for TB)
• Antiepileptics: Carbamazepine, phenytoin, phenobarbital
• Antidepressants: St. John’s Wort, fluvoxamine, nefazodone
• Blood Pressure Lowering Agents: Atenolol, amlodipine, enalapril, telmisartan, olmesartan

Contraindications

• Avoid use with strong CYP3A4 inhibitors.
• Not suitable for patients with adrenal insufficiency.

Pregnancy & Lactation

Pregnancy:

• There is no data on Firendia 10’s use during pregnancy to assess risks of major birth defects, miscarriage, or adverse outcomes. Animal studies show developmental toxicity at exposures approximately 4 times higher than human levels. The clinical significance is unclear. The general risk of birth defects and miscarriage in the U.S. population is 2-4% and 15-20%, respectively. Avoid Firendia 10 during pregnancy unless deemed necessary by a healthcare provider.

Breastfeeding:

• There is no data on the presence of finerenone in human milk or its effects on breastfed infants. Animal studies suggest that finerenone is present in rat milk and may be in human milk as well. Due to potential risks to the infant, avoid breastfeeding during treatment and for 1 day after finishing Firendia 10.

Hyperkalemia Risk with Finerenone

• Risk: Finerenone can cause hyperkalemia (high potassium levels), particularly in patients with reduced kidney function or elevated baseline potassium levels.
• Pre-Treatment: Measure serum potassium and estimated glomerular filtration rate (eGFR) before starting Finerenone. Do not initiate treatment if serum potassium is > 5.0 mEq/L.
• During Treatment: Regularly monitor serum potassium levels. Adjust the dosage as needed based on these levels. More frequent monitoring may be required for patients at higher risk of hyperkalemia, including those on other medications that affect potassium levels.
